Job Description
Job Title : Case Manager RN
Location : 1300 West Seventh Street, San Pedro, CA 90732

Duration : 13 weeks  
Shift: Day 5x8-Hour (08:30 - 17:00) (Some weekends)
Onsite: Yes

Job Description:
  • The case manager performs the primary functions of assessment, planning, facilitation and advocacy, which are achieved through collaboration with the client and other health care professionals involved in the client's care
  • Assess new patients
  • Update and revise patient health care plans as needed
  • Educate patients and families about health care plans and options
  • Make clinical decisions for routine patient care
  • Accurately document and submit medical records
  • Demonstrate knowledge in medical care; such as diagnostic procedures, medication, symptoms, and other treatment-related therapies
  • Provide emotional support to patients and families
  • Evaluate and determine needs of patients
  • Review results from medical tests
  • Implement physician orders and other nursing procedures
  • Create an environment for open communication
Qualifications:
  • 2 years of Acute Care experience (can consider 1.5 years)
  • Experience with Utilization Review and Discharge Planning
  • Experience with InterQual
Certifications:
  • California Registered Nurse License
  • BLS-Basic Life Support
